Appointments:

Champlain Obstetrics & Gynecology looks forward to our meaningful partnership starting on your first visit with us. Please call our Essex Junction, VT office at (802) 879-1802 to schedule your preferred appointment time. You can also use our online Request an Appointment form. If you are unable to keep your appointment please call us within 24 hours so that we can reschedule you for a more convenient time.

Once your initial appointment has been scheduled, you can preregister by filling out our online Patient Health History Form or download and print the forms below to bring with you on your first visit. Please complete these forms prior to your arrival so you can get in to the doctor's office immediately.
